Stupid forms are often accompanied by stupid statements, like this one from a bright lad in our Human Resources department:

"Only those people who receive the forms will be required to fill them out"

Once upon a time, the void was formless...

My favorite example of Stupid Form Questions is "Have you experienced
any memory deficits?"  If not, I'd answer in the negative.  If true,
I'd not remember them and would also check off "NO."  Joseph Heller
must be laughing somewhere!
